The Intersubjective Relational Field: Where the Nervous System Learns What Is Possible

What actually changes people? Psychotherapy has circled that question for decades, and every orientation answers it differently. When I was in graduate school I studied the work of Barry Duncan and Scott Miller as they explored the factors with the greatest influence on the change process in psychotherapy. They emphasized that client factors, the therapeutic alliance, and the client's perception of progress are among the strongest predictors of change. Duncan and Miller have often highlighted findings suggesting that approximately 40% of therapeutic outcome variance is associated with client factors and life circumstances, 30% with the therapeutic relationship, 15% with placebo, hope, and expectancy, and 15% with specific treatment models and techniques. And the debate goes on...
Cognitive therapies point to new ways of thinking. Behavioral therapies point to new learning through experience. EMDR points to adaptive information processing. Attachment theory points to relationship itself. Gestalt therapy points to awareness and authentic contact. Neuroscience has more recently added its own answer: the brain is not a passive recorder of experience but an active predictor of it.
I used to hear these as competing explanations.
These days I hear them as five different rooms describing the same house. What they are all pointing toward, I've come to think, is the intersubjective relational field. I no longer treat the therapeutic relationship as backdrop, and I no longer think of it as one “common factor” sitting alongside the others. It is the environment in which the nervous system decides whether change is even possible. Before a client can take in new information or try on a different way of being, something more basic gets settled first, not consciously, but physiologically: What can I predict will happen here?
I once worked with a client, I'll call her Maya, who checked my face for a beat before answering almost anything I asked her. Not dramatically. Just a half-second scan, then the answer. It took me months to name what I was watching: not caution, a prediction running in real time, built from a lifetime of rooms where honesty had cost her something. Once we could talk about the scan itself, rather than just what came after it, the work changed.
That's the pattern I now see everywhere in the room. Clients arrive with a lifetime of embodied predictions about relationships: who will misunderstand them, who will leave, which tone of voice means the ground is about to shift. We usually call these symptoms, defenses, or attachment strategies, and each label is useful. But underneath the label, I think they're predictions: the nervous system's best current guess about what's coming, based on what it's been taught to expect.
This lines up with what predictive processing researchers such as Karl Friston and Andy Clark have argued about perception more broadly: the brain isn't simply receiving the world, it's continuously forecasting it and checking the forecast against incoming sensation. Attachment theory said something similar long before neuroscience had the language for it. A child's earliest relationships become the template for what to expect from self and other. Attunement, inconsistency, neglect, fear, none of it just becomes memory. It becomes a predictive model that keeps organizing perception long after the original relationship is gone.
That reframes what we usually call resistance. I don't think it's opposition to change. I think it's the nervous system trying to stay coherent with the relational world it has always known, and if the brain runs on prediction, uncertainty can feel more dangerous than familiar suffering ever did. Seen that way, a client's adaptations aren't pathology. They're the nervous system doing exactly what it evolved to do: using the past to brace for the future.
This is part of why I keep returning to Gestalt field theory. Long before predictive processing had a name, Kurt Lewin argued that behavior is a function of the person and the environment together, a direct challenge to the idea that psychological life happens only inside one skull. Gestalt therapy took that further: experience is co-created at the contact boundary between organism and environment. Therapist and client are never separate from the field they're building together. Every moment of contact, withdrawal, or repair reorganizes what's possible within it.
Which means awareness was never a solo act. It emerges in relationship, in a room that is a living system rather than a delivery mechanism for interventions. Interpersonal neurobiology has started describing the same thing in its own vocabulary. Long before a client consciously decides whether to trust me, their nervous system is already gathering evidence: tone, pacing, the half-second of eye contact, and reaching a verdict about safety well before either of us has put it into words. Daniel Siegel's definition of the mind as “an embodied and relational process that regulates the flow of energy and information” is a neuroscientist's way of saying what Gestalt therapists have watched happen in the room for decades: healing starts between two people before either one of them understands it.
So when I ask what actually changes people, I no longer reach for a single mechanism. I reach for the field itself: the thing being built, moment by moment, between two nervous systems learning what's possible in each other's presence. Every modality I trained in turns out to have been describing a piece of it. The field was always the whole picture.
